Please arrive at the office 15 minutes early to fill out your New Patient Forms or have them printed out and completed before you even enter the office for a shorter wait time.
Two ways to be prepared for your visit and not have to fill out paperwork in the office:
- You may go to Patient Login Tab and fill out the forms online
- Go to Patient Login Tab and click on Patient forms and then print them from your computer.
Please have this form filled out by your physician before your appointment if you are on Plavix or Coumadin, have a preexisting heart condition, or have a medical condition that needs additional precaution. This will tell us if you need to be premedicated or if there are any necessary precautions our staff should take before your next visit.
